This plugin is unique in that it does not affect the default /wp-login.php /wp-admin or /wp-login.php?action=register pages. You may be experiencing an issue with another plugin or your current theme.

You can try accessing your login pages by one of the following URLs:
http://my-site.com/wp-login.php
http://my-site.com/wp-admin
http://my-site.com/wp-login.php?action=register

If those still fail, and you believe the plugin is causing an issue then you can FTP/SFTP into your server and remove the plugin files, then revisit your default wp-login.php page.
