We have just checked and we found out that you have wrapped the [ap-form] shortcode with
pre and code html tags.
That may have been occurred due to direct copy paste of shortcode from Plugin's "How to use" panel to visual version content editor.So please go to the editor section where you have pasted the shortcode and from text version editor, remove the
pre and code tags and the layout will properly show up.
